29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   The employer failed to protect employees from potential electrical, caught between and amputation hazards associated with performing maintenance on equipment requiring the de-energizing of stored energy without having developed and implemented a written lockout and tag out program.  This was most recently evident during the inspection of the facility located at 46 Hoolai Street, Kailua, HI 96734, where the employer has employees and outside contractors working on baking ovens without having a written lockout and tag out program in place to establish the necessary safety methods to prevent injury.  Instances include but are not limited to:    i.  A month ago the owner of the company performed maintenance on the oven without applying any locks to the circuit breaker to prevent the accidental restart of the oven with thermal and electric stored energy.     ii. A month and a half ago the employer had an outside contractor working on the big oven without having established whose written procedures shall be used to ensure that all stored energy is isolated.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  The employer failed to protect employees from potential amputation hazards associated with operating unguarded bakery mixers.  This was most recently evident during the inspection of the facility located at 46 Hoolai Street, Kailua, HI 96734, where the employer has employees operating the unguarded Hobart mixer model # 11-38-847, serial # 0-300 in the process to make cookie and cakes.
