29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    On July 7, 2013,  and times prior to, employees were working near gears and chains that powered a conveyour belt that was not fully guarded.  This conditon exposed employees to cuts and lacerations.

29 CFR 1910.303(f): Each disconnecting means for motors and appliances was not located and arranged so the purpose was evident, nor legibly marked to indicate its purpose.     On July 8, 2013, and times prior to, employees were using a conveyour electrical system that was not marked to indicate its purpose, such as turning on/off the equipment.  This conditon exposed employees to potential struck by and electrical hazards.
