29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:    a) On or about March 26, 2013, the employer did not ensure employees performing framing work used the top step of a ladder. The employees were exposed to an unprotected fall of over 6 ft. to the concrete below.
