29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied.  824 S. Frontage Rd., Scott, LA:  On or about May 6, 2013, the employer allowed an employee to work from basket/pallet elevated by a forklift.  The basket/pallet was not securely attached to the forklift and the forklift was mover horizontally with an employee in it, exposing the employee to a fall hazard of approximately 16 ft. to the ground.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems meeting the requirements of paragraph (g)(4) of this section.  824 S. Frontage Rd., Scott, LA:  On or about May 6, 2013, the employer failed to ensure that the aerial lift basket an employee worked from had a complete guardrail system.  The basket was missing the midrail.  The employees were working approximately 16 feet above the ground exposing them to a fall hazard.
