Frontage Rd., Scott, LA:  On or about May 6, 2013, the employer allowed an employee to work from basket/pallet elevated by a forklift.  The basket/pallet was not securely attached to the forklift and the forklift was mover horizontally with an employee in it, exposing the employee to a fall hazard of approximately 16 ft. to the ground.
