On or about May 07, 2013, at the above located workplace, employees in the histology laboratory were required to work with NBF-Clear and formalin solutions containing formaldehyde. No monitoring was performed by the employer to determine the employee exposure to formaldehyde.

29 CFR 1910.1048(i)(2): If employees' skin may become splashed with solutions containing 1 percent or greater formaldehyde, for example, because of equipment failure or improper work practices, the employer shall provide conveniently located quick drench showers and assure that affected employees use these facilities immediately. a. On or about May 07, 2013, at the above located workplace, histology laboratory employees required to work with NBF-Clear and formalin solutions containing 10% formaldehyde were not provided with suitable facilities for quick drenching of the body.

On or about May 07, 2013, at the above located workplace, employees in the histology laboratory were required to work with NBF-Clear and formalin solutions containing formaldehyde. The employer did not provide training to inform all laboratory employees of the hazards associated with formaldehyde.
