29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:     At the jobsite located approximately at 3339 N. 124th Street, Omaha, NE: Employees of Jose Zepeda Roofing were exposed to a fall height greater than 6 feet while installing asphalt shingle roof on a residence. The employer did not ensure that employees utilized 100% fall protection as required by the standard.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures           to be followed in order to minimize these hazards:    Jose Zepeda Roofing:  The employer did not have a training program to ensure that employees minimized hazards when exposed to fall hazards. Employees of Jose Zepeda were observed by a Compliance Officer performing roofing activities at heights greater than 6 feet without using any convention fall protection; none of the employees had received training from their employer.
