29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.   for the loading dock area at the southeast corner of the facility located at 2656 Waiwai Loop, Suite C, as observed on or about 05/03/2013.  The employees working for Cabinet Distributors, Inc. were exposed to 4-foot fall hazards due to missing or damaged guardrails.     a)  The open, upper edge of the dock had no chains or railings to prevent employees from falling four-feet into the bottom of the ramp.     b) The guard rails along the southeast edge of the ramp were rusted through and deteriorated so bad, they would not support the weight of an employee and allow them to fall 4-feet into the ramp, or be impaled by the exposed rebar holding part of the rail up.
