29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a) Throughout Establishment: On or about May 06, 2013 and at times prior thereto, employees were exposed to corrosive liquids such as but not limited to; Butoxyethanol, Aqua Ammonia (DEEP BLUE Glass Cleaner); and Hydrochloric Acid (Muriatic Acid) while using these chemicals to clean and maintain the establishment without proper eye and hand protection.

29 CFR 1910.1200(e)(1): Employers did not develop, implement, and maintain at each workplace, a written hazard communication program:  (a) Throughout Establishment: On or about May 06, 2013 and at times prior thereto, employees were exposed to corrosive liquids such as but not limited to, Butoxyethanol, Aqua Ammonia (DEEP BLUE Glass Cleaner), and Hydrochloric Acid (Muriatic Acid), while using the chemicals without a set hazardous communication program to identify labeling, material safetey data sheets, and employee training.
