29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: The employer failed to protect employees from potential electrical, caught between and amputation hazards associated with performing maintenance on equipment requiring the de-energizing of stored energy without having developed and implemented a written lockout and tag out program. This was most recently evident during the inspection of the facility located at 801 Kaheka St. Honolulu, HI 96814, where the employer has employees and outside contractors working on equipment within the facility without having a written lockout and tag out program in place to establish the necessary safety methods to prevent injury. Instances include but are not limited to: i. Employees changing the saw blade on a Hobart vertical band saw used to cut meat portions. ii. Contractors making repairs to the Hobart vertical mixer. iii. Contractors making repairs to the Hobart baking ovens. iv. Contractors making repairs to the Hobart Meat Grinder.

This was most recently evident during the inspection of the bakery at the facility located at 801 Kaheka St. Honolulu, HI 96814, where the employer has employees using the Hobart vertical mixer and the electrical disconnect is blocked by a storage rack making it infeasible to turn off the main power in the event of a machine malfunction.

This was most recently evident during the inspection of the meat department at the facility located at 801 Kaheka St. Honolulu, HI 96814, where the employer has employees using the Butler model # 120, serial # 3946 to make hamburger patties from ground beef and the machine is plugged into a temporary flexible cord that has been fixed permanently in lieu of standard electrical conduit.
