At building 1260, employees were exposed to slip and fall hazards up to 20 feet above the ground level when accessing the roof using a permanent, fixed ladder with the following deficiencies:    A).  The rungs were not coated with non-slip coating.    B.)  The top rails were made of angle iron which projected into the climbing path.
