29 CFR 1910.27(b)(1)(ii): The distance between rungs exceeded 12 inches and were not uniform throughout the ladder:    At building 1260, employees were exposed to fall hazards  as they climbed up and down a fixed ladder with its ladder rungs spaced 16.5 inches between rungs.

29 CFR 1910.27(f): All ladders were not maintained in a safe condition. All ladders were not inspected regularly, with the intervals between inspections being determined by use and exposure.    At building 1260, employees were exposed to slip and fall hazards up to 20 feet above the ground level when accessing the roof using a permanent, fixed ladder with the following deficiencies:    A).  The rungs were not coated with non-slip coating.    B.)  The top rails were made of angle iron which projected into the climbing path.
