Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee(s) that were exposed to crushing hazards:     (a) Location - 16 Oxbow Street, Lexington, MA   On or about 5/2/13, an employee was exposed to a rollover hazard from a dump trailer that was not properly operated.  Ground was not firm and level and only one of the trailer gate-locks was opened prior to lifting the trailer.
