(a) Southwest Roofing, LLC on or about May 08, 2013, at the address of 519 Walnut St. Elyria, OH: The employer did not ensure employees were protected from head injuries while working below employees conducting roofing work above. Employees were exposed to potential impact injuries caused from falling material and/or equipment.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  a)  Southwest Roofing, LLC on or about May 08, 2013, at the address of 519 Walnut St. Elyria, OH: The employer did not ensure employees were protected from falls while installing new roofing material to a residential type structure.  Employees were exposed to a 24 ft. fall to the pavement below.
