(2)  The extension cord was damaged in that the male end had the insulation pulled from the cord connector end.   The Jose Family Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number312345572, citation number 1, item number 3a and was affirmed as a final order on 08/25/2010, with respect to a workplace located at 35-37 Berkley Street, Providence, RI.
