29 CFR 1926.55(a): Employee(s) were exposed to material(s) at concentrations above those specified in the Threshold Limit Values of Airborne Contaminants for 1970 of the American Conference of Governmental Industrial Hygienists:   a.) Giant Supermarket, Meat Department - An employee, who was assisting with the cutting of the concrete floor in an enclosed area was exposed to carbon monoxide at an 8hr time weighted average of 125.3 parts per million(ppm).  This is over 2.5 times the Permissible Exposure Limit (PEL) of 50 ppm for carbon monoxide.  Exposure occurred over a 90 minute period on May 05, 2013. A zero exposure was assumed for the remaining 390 minutes.   b.) Giant Supermarket, Meat Department - An employee, who was assisting with the cutting of the concrete floor in an enclosed area was exposed to carbon monoxide at an 8hr time weighted average of 74.5 parts per million(ppm).  This is over 1.5 times the Permissible Exposure Limit (PEL) of 50 ppm for carbon monoxide.  Exposure occurred over a 90 minute period on May 05, 2013. A zero exposure was assumed for the remaining 390 minutes.   c.) Giant Supermarket, Meat Department - An employee, who was using a Core Cut CC3500 propane powered Concrete cutting saw to cut concrete flooring in an enclosed area was exposed carbon monoxide at an 8hr time weighted average of 186.3 parts per million(ppm).  This is over 3.7 times the Permissible Exposure Limit (PEL) of 50 ppm for carbon monoxide.  Exposure occurred over a 90 minute period on May 05, 2013. A zero exposure was assumed for the remaining 390 minutes.

Giant Supermarket, Meat Department - The employer did not implement feasible administrative or engineering controls to reduce employee exposure to below the Permissible Exposure Limit (PEL), nor demonstrate that such controls were not feasible for an employee, who was assisting with the cutting of the concrete floor in an enclosed area and who was exposed to carbon monoxide at an 8hr time weighted average of 125.3 parts per million(ppm).
