29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about May 7, 2013, on the north side of the work site, an employee was exposed to a fall hazard of approximately 8 feet while performing carpentry work on a commercial building without fall protection.

On or about May 7, 2013, on the north side of the construction site, an employee working on a commercial building roof  utilized an extension ladder that did not extend 3 feet above the landing exposing the employee to a fall hazard.
