Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about May 9, 2013, during the installation of rubber membrane roof material and other roofing work at a height of approximately 30 feet, the employer did not ensure employees were protected from fall hazards by ensuring a fall protection system system/device was being used.
