29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    On or about May 9, 2013, during the installation of rubber membrane roof material and other roofing work at a height of approximately 30 feet, the employer did not ensure employees were protected from fall hazards by ensuring a fall protection system system/device was being used.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:     On or about May 9, 2013, the employer did not ensure employees using the existing stairways in the building were provided at least one handrail on stairways having four or more risers. Employees were thereby exposed to falls while ascending and descending the stairways to the second and third floors to perform work activity.
