29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about May 9, 2013, the employer failed to ensure that an energy control program (LOTO) was developed and implemented for machines throughout the shop with more than one power source that require maintenance or servicing.  Some of the equipment includes but is not limited to, milling machines, mills, lathes, hydraulic press, plate shears and plate rollers.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   The following equipments in the manual shop were not provided with the required guarding to prevent contact with the rotating parts:  a)  Manual lathes #3 with 18 inch chuck (serial no. unclear)  b)  Webb milling machine, model #TW/40/2IF, mfg. #9510971  Employees were exposed to rotating and moving parts.

Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident.  Machine shop, west wall:  Three electrical panel boxes were not labeled to indicate what equipment each breaker supplied power to.  Employees were exposed to the hazard of electrical shock.
