29 CFR 1910.27(c)(6):  The step-across distance from the nearest edge of ladder to the nearest edge of equipment or structure was greater than 12 inches.    a)  Baler #2 -  Employees used the ladder and 18 foot high walkway about 1-2 times a month to remove jams in the machine.  The step across distance from the ladder to the leading edge of the walkway was 31 inches, therebty exposing the employee to a hazard, observed on or about 5-9-13.    Abatement certification required.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a) Baler Areas - The baler operators use dust masks when they clean out the paper from under the baler machine  conveyor but they had not been provided with Appendix D to 29 CFR 1910.134, reported on or about 5-7-13.  Abatement certification required.
