1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:      Employees were exposed to a fall hazard up to 8 feet 4 inches while removing roofing material and no fall protection was being used.    Doug Heinz dba Heinz Roofing was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard: 29 CFR 1926.501(B)(13) which was contained in OSHA inspection number 312890312, citation number 1, item number 2A AND WAS AFFIRMED AS A FINAL ORDER ON  January 5, 2010 with respect to a workplace located at 13321 n Bayberry Ct. Chillicothe IL, 61523.

There was no written documentation that included the employees name, date of the training, and the signature of the person who conducted the training or the signature of the employee.
