29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: SSC Palisade Operating Company LLC, dba Palisades Living Center, at 151 East 3rd Street, Palisade, CO: On and preceding 5/8/13, Palisades Living Center did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from liquid chemicals, acids or caustic liquids. Dietary employees cleaned work surfaces in the kitchen of the nursing home. Employees used EcoLab Oasis 146 Multi-Quat Sanitizer when cleaning surfaces in the kitchen. The employer did not ensure that employees used eye protection when cleaning with liquid chemicals. This condition exposed employees to a chemical hazard.

29 CFR 1910.1030(d)(2)(vii)(A): Contaminated needles and other contaminated sharps were bent, recapped, or removed: SSC Palisade Operating Company LLC, dba Palisades Living Center, at 151 East 3rd Street, Palisade, CO: On and preceding 5/8/13, Palisades Living Center did not ensure that contaminated needles and other contaminated sharps were not bent, recapped, or removed. Caregivers used disposable razors to shave residents of the nursing home. After use, the contaminated sharp was recapped prior to disposal into a sharps container. The employer did not ensure that contaminated sharps were not recapped. This condition exposed employees to a bloodborne pathogen hazard.

29 CFR 1910.1030(g)(2)(vii)(F): The bloodborne pathogens training program did not contain an explanation of the use or limitations of methods that would prevent or reduce exposure including appropriate engineering controls, work practices or personal protective equipment: SSC Palisade Operating Company LLC, dba Palisades Living Center, at 151 East 3rd Street, Palisade, CO: On and preceding 5/8/13, Palisades Living Center did not ensure the bloodborne pathogens training program contained an explanation of the use or limitations of methods that would prevent or reduce exposure including appropriate engineering controls, work practices or personal protective equipment. Nurses used Kendall Monoject syringes, Ulticare lancets, Lantus SoloStar insulin injectors, IV systems, and blood collection devices while caring for residents of a nursing home. The employer had determined that the nurses were occupationally exposed to blood and/or other potentially infectious materials (OPIM). Needlesticks have occurred in the workplace. The employer did not provide annual bloodborne pathogens training with regards to the medical devices used by nurses. This condition exposed employees to a bloodborne pathogen hazard.
