29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Dough Processing Area: The employer had not established a lockout/tagout program when the employees were performing servicing, cleaning and maintenance of bakery equipment such as, but not limited to, dough separators, dough mixers and ovens.

29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed in accordance with 29 CFR 1910.147(f)(1)(i)-(v): (a) Dough Processing Area: Employees performing cleaning of the Turu Dough Separator in the inch mode were not provided with a sequence of actions to follow in accordance with 29 CFR 1910.147(f)(1)(i)-(v) when energizing the equipment.
