(a) Jobsite - On or about May 13, 2013 an employee was operating a Toyota forklift without being trained and evaluated on how to operate forklift safely.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition.      (a) Jobsite - On or about May 13, 2013, three forklifts were found to be in need of servicing:    (i) Taylor T180 forklfit had an inoperable horn and a damaged rear right tire  (ii) Taylor T200 forklift had an inoperable horn and lights and  a damaged front lift tire  (iii) Toyota forklift had an inoperable right headlight

29 CFR 1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations:     (a) Framing Ramp - On or about May 13, 2013 ground prongs were broken off and lodge inside two electrical outlets causing an electric shock hazard to employees using the outlets.     (i) outlet on the pole underneath the framing shed   (ii) outlet on the outside of the equipment storage shed
