29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. International Bridge - An employee climbed 34 inches of rebar and concrete and a ladder or stairway was not utilized.

29 CFR 1926.20(b)(3): The use of any machinery, tool, material, or equipment which is not in compliance with any applicable requirement of this part is prohibited. Such machine, tool, material, or equipment shall either be identified as unsafe by tagging or locking the controls to render them inoperable or shall be physically removed from its place of operation. International Bridge - The five riser stairway, missing a handrail, was not posted with a danger sign, tape or tag.
