29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:   a)  I-84 West, Asylum Street, exit 48 - On or about 5-15-13 - Employee was using a ladder to dismantle concrete form work panels and was working sideways on the ladder.  The employee was not facing his work, employees center of gravity was over the side rail of the ladder which caused the ladder to slip and fall.  As per manufacturers instruction the ladder is not designed for side loading by employee.
