29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage firefighting:     a)  Pet360 Inc. - The employees handles product that is packaged in Class A combustible material and were not provided an educational program to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage firefighting, exposing the employees to burn or smoke inhalation hazards on or about May 7, 2013.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   a)  Pet360 Inc. - On or about May 7, 2013 employees, electric forklift operators, were not provided training nor evaluated in powered industrial truck operation exposing them to safety hazards including the unsafe handling of loads.      Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  a)  Pet360 Inc. - On or about May 7, 2013, employees were exposed to electrical shock hazards from a damaged electrical outlet number 4A that powered a 120 volt air cushion machine at packing tables 3/4.    b)  Pet360 Inc. - On or about May 7, 2013, employees were exposed to electrical shock hazards from a damaged electrical outlet number 2B that powered a 120 volt air cushion machine at packing tables 1/2.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    a)  Pet360 Inc. - Packing Dept. - On or about May 7, 2013, employees were exposed to electrical shock and fire hazards from extension cords used in lieu of permanent wiring to power ten (10) 110 volt air packing machines.  This includes quilting and/or cushioning machines at Tables 1 through 12.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
