29 CFR 1910.252(a)(2)(i): Wherever there were floor openings or cracks in the flooring that could not be closed, precautions were not taken so that no readily combustible materials on the floor below would be exposed to sparks which might drop through the floor:   On or about 01/04/2013, oil soaked absorbent mats, located in a trench under the floor grates at D-2225, were not adequately guarded. Slag from overhead tack welding, contacted the absorbent mats resulting in a small fire.

29 CFR 1910.252(a)(2)(xv): Cutting or welding was permitted in areas that were not made fire safe by removing combustibles or protecting combustibles from ignition sources:   On or about 01/04/2013 the employer failed to ensure that any and/or all potential ignition sources were removed before proceeding with welding activities.  Slag from overhead welding operations, contacted an oil soaked absorbent mat, creating a small fire.
