29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval; and capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly:     Establishment - The employer attached a Long Reach slip sheet machine to a Model FC4515-50 Crown lift truck without prior written approval from the lift truck manufacturer, and without attaching a capacity plate specific to the use of the attachment on the lift truck.
