29 CFR 1926.501(b)(13):   Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   Job site located at 179th and Corby Streets, Omaha, NE:  Employees were observed conducting roofing activities on a residential structure.  The employees were exposed to falls of up to 25 feet and not less than 12 feet without using any form of fall protection such as a personal fall arrest system.     Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.1053(b)(1):   Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder�s length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.  Job site located at 179th and Corby Streets, Omaha, NE:  An extension ladder used to access a roof surface didnt have the side rails extended at least 3 feet above the upper landing surface (roof).  The ladder was extended 18 inches above the surface and had been used by 9 employees.     Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.
