29 CFR 1910.132(h)(1): Personal protective equipment (PPE), was not provided by the employer at no cost to employees.   On or about May 15, 2013, employees were required to either purchase leather gloves from the employer or purchase gloves from a vendor of their choice and at their cost.  This condition exposed employees to laceration, burn, and skin irritation hazards from using excessively worn gloves as a cost savings measure, or from selecting a glove inappropriate for the hazard.

29 CFR 1910.138(b): The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified:   On or about May 15, 2013, employees were exposed to contact with metalworking fluid that permeated their leather gloves.  The gloves were worn for protection when handling metal pieces for work conducted at the band saw.

29 CFR 1910.133(a)(5): The employer did not ensure that each affected employee used equipment with filter lenses that had a shade number appropriate for the work being performed for protection from injurious light radiation:  On or about May 15, 2013, a plasma cutting operator was provided shaded safety glasses not rated for protection against the light generated by the cutting of mild steel.  This condition exposed the employee to injurious light capable of causing photokeratitis (welder's flash).
