29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   The employer does not ensure that energy control procedures were developed for the control of potentially hazardous energy.  This violation was observed on or about May 15, 2013 in the facility where operators and maintenance employees were exposed to struck-by and caught-in hazards when performing set-up or maintenance tasks on lathes, band saws and other machines such as but not limited to;   a. CNC Lathe, Clausing Model T4, #R900011   b. CNC Lathe, Clausing Model T4. #R90011  c.  CNC Lathe, Clausing Model 200 #C20549   d.  CNC Lathe, Clausing, Model 100 #C10388   e. Hensaw Band saw Model H90A-B/F #867204  f.  Kaiser Air Screw Machine #1233  g. Hensaw Band Saw-Model H90A-4 #1119112  H. Bar Beveler #98032  I.  CNC Mill, Hass #1094681    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that it has procedures developed, documented and utilized for the control of potentially hazardous energy.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:   The employer does not ensure that flexible cords and cables were connected to devices and fittings that would prevent pull from being directly transmitted to joints or terminal screws. This violation was observed on or about  May 15, 2013 at the plant where employees were exposed to electrical hazards on a Harding Polishing Machine power cord where the outer insulation had been pulled back from the plug.         Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that flexible cords and cables were connected to devices and fittings that would prevent pull from being directly transmitted to joints or terminal screws.
