29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a. On or about May 15, 2013, at the northeast side of building No. 5, one employee using an air nail gun to install truss braces was not wearing eye protection, exposing himself to being struck by flying debris or nails. b. On or about May 15, 2013, at the northwest side of building No. 5, one employee using an air nail gun to install truss braces was not wearing eye protection, exposing himself to being struck by flying debris or nails.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. On or about May 15, 2013, at the northeast side of building No. 5, one employee installing truss braces on a 4:12 pitch roof was not protected from a 14 feet fall hazard by the use of conventional fall protection system or alternative fall protection measures. b. On or about May 15, 2013, at the northwest and southwest side of building No. 5, two employees installing truss braces on a 4:12 pitch roof were not protected from a 10 feet fall hazard by the use of conventional fall protection system or alternative fall protection measures.
