29 CFR 1926.20(b): Accident prevention responsibilities. 29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. a. On the site where the employer was removing and replacing electrical heaters and electrical systems, there was no competent person on site to identify hazards relating to falls, and electrical thereby exposing employees to a trip fall hazard.

29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a. In Building 4 where employees were performing electrical work, the electrical out covers were removed exposing live energized parts when employees plugged in portable power tools thereby exposing employees to a contact hazard. b. In the Social Hall where electrical equipment was removed, the face plates were removed and not replaced where employees plugged in portable power tools thereby exposing employees to contact hazard.

29 CFR 1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means. a. On the site where an employee was removing electrical outlet boxes as part of a remodeling project, the electrical power was not shut off before the work was done thereby exposing employees to a contact hazard.
