On or about March 6, 2013, the hatchway opening for the fixed inside ladder used to access the roof was not equipped with standard railings , closed cover or equivalent protection to prevent employees from falling through the opening once on the roof, and when pulling tools and supplies up through the hatchway by rope cable while standing on the roof near the opening. 29 CFR 1910.27(d)(3): The employer did not ensure that the side rails of a through ladder extended 3 1/2 feet above parapets and landings, omitting the rungs from the extension and having not less than 18 nor more than 24 inches clearance between the rails.    Inside roof access.  On or about March 6, 2013, the fixed ladder used to access the roof through the open hatchway did not have side rails (or equivalent protection) that extended 3 1/2 feet above the roof landing level for employees to hold onto to prevent falls to the floor below when climbing from the ladder onto the roof. 29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energization, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   Entire Plant.  On or about the weeks of February 25, 2013 and March 4, 2013, employees performed servicing and maintenance (including setup) on machines or equipment with multiple energy sources such as but not limited to conveyors, palletizers, de-palletizers, mixers, capper machine, tipper machine, compressors, injection molding machines, riveting machines, paper cutter and die cutter.  The employer did not implement and maintain a lockout/tagout program consisting of specific energy control procedures, employee training and periodic inspections as follows:    a) Procedures for the control of hazardous energy were not developed, documented and utilized by the employer.    b) Training of "Authorized", "Affected" and "Other" employees was not conducted and certified by the employer.   Employees authorized to perform lockout/tagout were not informed of and trained to control hazardous energy sources according to specific procedures.   c) Periodic inspections of the energy control procedures were not conducted and certified by the employer at least annually to ensure that the procedures and requirements of the standard were being followed.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
