On or about 5/17/2013 an employee was observed working from a Genie Articulating Boom lift model Z- 45/25 without wearing a body belt and lanyard and anchored to the boom or basket.

On or about 5/17/2013, An employee was observed working without fall protection while repairing a concrete cap on the side of a commercial building approximately 25 feet from ground level.

On or about 5/17/13 an employee working for Northern Rail Services, Inc. was observed working on a roof exposed to fall hazards had not received training to recognize and the procedures to be followed in order to minimize those hazards.
