29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    A.)  American Hollow Boring Company; main machine shop north - On or about 5/17/13, the lathes (#40 Lehmann) had a lathe head and a point of operation which created flying chips.  The lathe areas where the flying chips were created was not guarded.      B.)  American Hollow Boring Company; main machine shop north - On or about 5/17/13, the lathes  (#40 Lehmann) had a lathe head that turned.  The rotating part was not guarded.    C.)  American Hollow Boring Company; main machine shop north - On or about 5/17/13, the boring machines (#1) had a boring head and a point of operation which created flying chips.  The boring area where the flying chips were created was not guarded.      D.)  American Hollow Boring Company; main machine shop north - On or about 5/17/13, the boring machines (#1) had a boring head that turned.  The rotating part was not guarded.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:   A.)  American Hollow Boring Company; warehouse, Panel H - On or about 5/17/13, a 55 gallon drum of scrap was located  17 inches in front of an electrical panel box.  The panel box was ued to power the lights in the facility.
