29 CFR 1926.1413(a)(2)(i)(A): 29 CFR 1926.1413(a)(2)(i)(A): The employer did not inspect wires to look for significant distortion of the wire rope structure such as kinking, crushing, unstranding, birdcaging, signs of core failure or steel core protrusion between the outer strands.    At the company dock on Pelican Island in Galveston, Texas, on or about May 20, 2013, employees operated a BIG T 550 ton barge crane, where the boom hoist cable was crushed in 12 different locations, exposing employees to struck-by and caught-in-between hazards.

At the company dock on Pelican Island in Galveston, Texas, on or about May 20, 2013, employees operated a BIG T 550 ton barge crane, where the boom hoist cable was crushed in 12 different locations, exposing employees to struck-by and caught-in-between hazards.
