On or about May 21, 2013, the employer failed to provide a seatbelt in the Clark C500-45 Type G forklift (Serial #C500-45-49-2520) located in the southeast section of the building used to hoist the powders used for mixing.  The employees were exposed to a crushing hazard if the forklift were to roll over.

On or about May 21, 2013, the employer allowed the voluntary use of 3M 8210 N95 respirators in the grinding area without providing them with the information contained in Appendix D.  Employees were observed using respirators while using a Makita GA7021 grinder to remove excess material from sink vanities.
