On or about May 21, 2013, the employer failed to provide a seatbelt in the Clark C500-45 Type G forklift (Serial #C500-45-49-2520) located in the southeast section of the building used to hoist the powders used for mixing. The employees were exposed to a crushing hazard if the forklift were to roll over.
