The workers were exposed to an electrical shock, burn or electrocution.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:      JOB SITE: 2139-2143 McKelvey Road, Maryland Heights, MO 63043, at this work site, a worker was observed tapping electrical power from a fuse box and the 120-volt, single-phase, 15 and 20 ampere receptacle outlets was not connected an approved ground fault circuit interrupters.  The workers were exposed to an electrical burn, shock or electrocution hazard.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage:   JOB SITE: 2139-2143 McKelvey Road, Maryland Heights, MO 63043, at this work site, the employer failed to provide an assured equipment grounding conductor program to ensure that each cord set plug and receptacle of equipment were visually inspected for external defects prior to its use.  The employer allowed his workers to use the gutter bending machine electrical plug with exposed wires and missing ground pin to the flexible cord.  The workers were exposed to an electrical shock, burn, or electrocution hazard.
