29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: The Dollar General Store #10836 was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 314300492, citation number 1, item number 4 and was affirmed as a final order on December 7, 2010, with respect to a workplace located at 405 East Braker Lane, Austin, TX 78753. On or about May 21, 2013, and at times prior thereto, at the store #11579, the employer did not ensure that employees had a clear and unobstructed path to exit doors. This practice exposed employees to the hazards of delay in emergency escape. Instance a: The means of egress to the west exit door located in the receiving room area was obstructed by trash bags, merchandise, boxes and containers of merchandise. Instance b: The means of egress to the east exit door located in the rear of the store was obstructed by a large cooling fan.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: The Dollar General Store #10836 was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 314300492, citation number 1, item number 3 and was affirmed as a final order on December 7, 2010, with respect to a workplace located at 405 East Braker Lane, Austin, TX 78753. On or about May 21, 2013, and at times prior thereto, at the Dollar General store #11579, the employer did not ensure that the portable fire extinguisher was mounted in a location that was readily accessible to employees without subjecting the employees to injuries. The fire extinguisher was mounted on the west wall adjacent to store front entrance and obstructed by such items as but not limited to store merchandise, floor standing commercial fan, merchandise cart loaded with approximately 14 - 25 pound bags of pet food, plastic yard chairs for adults and children, stackable plastic totes with merchandise, etc. This condition exposed employees to trip hazards during fire emergency conditions.

The Dollar General Store #10836 was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(g)(1)(ii), which was contained in OSHA inspection number 314300492, citation number 1, item number 2 and was affirmed as a final order on December 7, 2010, with respect to a workplace located at 405 East Braker Lane, Austin, TX 78753. On or about May 21, 2013, and at times prior thereto, at the Dollar General store #11579 receiving room area, the employer did not ensure an open working space was provided around enclosed live parts for inspection or servicing purposes. The electrical cabinets located in the receiving room area / west end were surrounded by items including, but not limited to 50 gallon plastic trash cans, plastic totes with and without store merchandise, wheeled merchandise containers with merchandise, several 14 pound bags of pet food, several 12 pack cardboard crates of glass Mason jars and full plastic trash bags. This condition prevented employees from accessing and inspecting the electrical panel.

On or about May 21, 2013, and at times prior thereto, at the Dollar General Store #11579 receiving room area, the employer did not ensure the mop sink floor basin was kept clean or in a sanitary condition. Inside the mop sink floor basin, stood 3 inches of black colored water. This condition created an environment which could breed bacteria.
