29 CFR 1926.20(b)(1): The employer did not initiate and maintain an accident prevention program as may be necessary to comply with the General safety and health provisions covered in 29 CFR 1926.20.   On or about May 16, 2013, the employer did not provide a workplace were any employee, laborer, or worker employed in the performance of the contract was free from recognized hazards exposing employees and workers to the following hazards:  a. In the interior of the building, workers were using electrical powered tools and extension cords with standing water on the concrete floors, exposing workers to electrocution hazards;  b. In the interior of the building on the concrete floors, workers were exposed to slipping hazards where standing water and mud had accumulated;  c. At the rear of the building, workers installing exterior framing and Tyvex weather protection from the concrete sidewalk were exposed to a fall hazard of approximately 7 feet into the unprotected excavation;  d. At the rear of the building, workers installing exterior framing and Tyvex weather protection were exposed to a fall hazard of approximatley 7 feet where the concrete sidewalk was undermined with sloughing of the excavation walls approximately 2 feet under the concrete working surface and evidence of continued cracking;  e. At the rear and front of the building where workers performing concrete work and steel erection activities were exposed to a fall hazard of an unguarded/barricaded excavation approximately 120 feet long, 60 feet wide and approximately 7 feet deep.
