29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.   a) Jobsite: First floor - On or about 5/20/13, employees were exposed to a fall hazard while conducting demolition activities around an unfinished staircase opening  that was not covered or guarded.  The hole measured approximately six feet in length, 2 feet in width, and 7 1/2 feet in depth.
