The subcontractor foreman was exposed to serious head injuries from falling materials and tools while picking up debris directly below a crew performing roofing work. Workers were exposed to a fall of approximately 22 feet to the ground below while using an aluminum extension ladder that did not extend above the eave at least 3 feet.
