OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:    a) Caremedix LLC, Arlington Heights- On or about April 2013, the employer used a working platform that did not meet the requirements for elevating personnel.  The employer was raised on the forks of a Toyota Electrical Powered Industrial Truck, Model #5FBCU25, Serial #64237, approximately 6 feet above the ground without fall protection.         Among other methods, one feasible and acceptable means of abatement to correct this hazard is to conform to the American Society of Safety Engineers (ASME), Safety Standard for Low Lift and High Lift Trucks. ANSI/ITSDF B56.1-2009, paragraphs 4.17.2 and 4.17.3.
