29 CFR 1926.200(g)(1): The employer did not provide the requisite advance warning of construction work being conducted on the traveled way:    (a) Altus Traffic Management, LLC at Federal Blvd. and 8th Ave., Denver, CO 80204: On May 22, 2013, the employer did not provide the requisite advance warning sign to warn road users of the right lane closure on Federal Blvd. heading southbound before 8th Ave.  This condition exposed employees in the work zone to a struck-by hazard.    Abatement Note: Among other methods, a feasible and acceptable abatement method to correct this hazard is to ensure proper placement of the advance warning sign. This abatement method is depicted in Part VI of the Manual on Uniform Traffic Control Devices (MUTCD) Millennium Edition in Figure 6H-21, Lane Closure on Near Side of Intersection (TA-21).

29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (MUTCD):   (a) Altus Traffic Management, LLC at Federal Blvd. and 8th Ave., Denver, CO 80204: On May 22, 2013, the employer did not follow the MUTCD to provide proper signs and devices with the required taper length for the right lane closure on Federal Blvd.  This condition exposed employees in the work zone to a struck-by hazard.
