29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Between the 1st and 2nd Floors - A foreman and carpenters were observed walking past an unguarded stair opening exposing them to a 10 foot fall hazard. b) North Side of the Building - A foreman and carpenters were observed using an unguarded the ramp to enter and exit the building exposing them to a 13 foot fall onto the basement patio deck. c) South Side of the Building - A foreman and carpenters were observed working near an unguarded window opening exposing him to a 31 foot fall hazard.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: a) South Side of the Building, 2nd Floor - A foreman was observed on an 8 foot step ladder, not opened, which was placed against a wall stud exposing him to a 42 inch fall hazard inside the building and a 31 foot fall hazard outside the building.
