29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used. On or about May 20, 2013, on the north side of the construction site, employees were exposed to electrical hazards while engaged in framing activities on a commercial building: a) The extension cord energizing the skill saw, had it's outer sheath burned and split exposing internal wires, and b) The power cord of the skill saw used to cut framing material was spliced.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about May 20, 2013, on the north side of the construction site, an employee was exposed to a fall hazard of approximately 20 feet while performing framing activities on a commercial building.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured. On or about May 20, 2013, on the north side of the construction site, an employee performing framing activities on a commercial building utilized an extension ladder that did not extend 3 feet above the landing.
