29 CFR 1910.1200(h)(1):  The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area  Forever 21, Inc/Bridgewater, NJ facility- Fitting Room:  The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment. Employees are using products such as but not limited to:  - 70% isoproply alcohol - Nail Polish remover (which contains acetone) - Comet (which contains calcium carbonate, sodium carbonate, and calcium hydroxide)  Condition observed on/or about May 23, 2013.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  Forever 21, Inc/Bridgewater, NJ facility- Fitting Room: The employer did not ensure that portable fire extinguiser readily accessible to employees in case of fire. Portable fire extinguisher was obstructed by boxes and access to it was obstructed.  Condition noted on or about May 23, 2013.  Forever 21, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 315641373, citation number 1, item number 2, and was affirmed as a final order on 5/11/2012,with respect to a workplace located at 400 Commons Way, Bridgewater, NJ.
