29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:  a)  1132 Main Street, Fishkill, NY - On and prior to May 22, 2013, employees were exposed a fall hazard of approximately 20' to ground level when performing masonry work from prefabricated frame scaffolding due to the platform ends extending over the supports up to 28" on the first, second and third levels.

29 CFR 1926.452(c)(4): End frames of tubular welded frame scaffolding were not locked together vertically by use of pins or equivalent means in areas where uplift could occur:  a)  1132 Main Street, Fishkill, NY - On and prior to May 22, 2013, employees were exposed to a fall hazard of approximately 18' to ground level when performing masonry work from prefabricated frame scaffolding that was missing a pin to lock tubular welded framing together.  The scaffolding was subjected to uplift due to the use of a cantilever platform on the front side of the scaffolding.
