29 CFR 1904.7(b)(3): The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent.   (a) US Border Patrol Douglas Station, 1608 S Kings Hwy, Douglas, AZ 85607: On or about 16 May 2013, and at times prior thereto, the employer incorrectly recorded the following work-related injuries and illnesses on the OSHA Form 300 for Calendar Year 2013:  Instance # 1:    In all recorded cases (33 total cases between 1 Jan 2013 and 10 May 2013) column (E) was incorrectly filled out, where the specific information on the location where the event occurred is missing. All cases were simply noted as as DGL AOR for the location.  Instance # 2:     In all 33 recorded cases no entry was made in column (A) specifying a distinct case number for each case.    Instance # 3:     In 4 cases (of the total 33 cases recorded), the number of days were not recorded for injuries which resulted in days away from work or days of restricted or job transfers. To wit:                         (a) event dated 1/15/13                          (b) event dated 2/24/13                         (c) event dated 3/18/13                         (d) event dated 5/10/13  Instance # 4:    In 28 (of 33 total cases) the entries recorded for column (F) did not specify the object or circumstance that directly injured or made the employee ill. These cases simply listed the part of the body affected.                                ABATEMENT DOCUMENTATION AND CERTIFICATION REQUIRED

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  (a) US Border Patrol Douglas Station, Bldg A, Dispatch Room, 1608 S Kings Hwy, Douglas, AZ 85607:  On or about 16 May 2013, and at times prior thereto, the employer did not ensure that each disconnecting switch in Panel 2 UA were identified as to location and purpose on the legend posted on the inside door cover, exposing employees to hazards.
