29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about May 23, 2013, excavation site, employees working on one end of The L-shaped excavation/trench (approximately 30-feet in total length) did not have cave-in protective equipment. The excavation section, in which employees were working, was 7-foot deep, 7-foot wide and 8-foot 6-inches in length. This condition exposed employees to crush by cave-in hazards.

29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary: On or about May 23, 2013, excavation site, the L-shaped excavation/trench approximately 30-feet in total length had the spoil piles set too close to the edge. This condition exposed employees to spoils and equipment rolling back on top of workers and cave-in hazards. a) The excavation section, in which employees were working, was 7-foot deep, 8-feet 6-inch in length, 7-foot wide had the spoil pile set 21-inches from the edge. b) The trenched section (the only egress passageway) was 15-feet long, 4-feet wide; 5-foot 4-inches deep had the spoil pile set at the edge.

29 CFR 1926.651(c): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: On or about May 23, 2013, excavation site, employees working on one end of the L-shaped excavation/trench (approximately 30-feet in total length) did not have a safe means of egress within 25 feet from where they were working. This condition exposed employees to fall hazards.
